Pelvic Health Care That Sees You, Respects You, and Supports Your Goals
You deserve healthcare where you can feel comfortable being yourself.
At Magic City Physical Therapy, we provide gender-affirming pelvic floor therapy in Hoover, Alabama, and the surrounding Birmingham metro for transgender, nonbinary, gender-diverse, and gender-questioning individuals. We welcome people at every stage of their journey, whether you are considering gender-affirming care, currently transitioning, preparing for or recovering from surgery, or simply looking for a pelvic health provider who understands and respects your experience.
Pelvic health is deeply personal. We understand that discussing your body, anatomy, sexual health, bladder or bowel function, or previous medical experiences can feel vulnerable. Our goal is to create a private, judgment-free, and supportive environment where you can ask questions, establish boundaries, and receive care that is individualized to you.
What Is Gender-Affirming Pelvic Floor Therapy?
Gender-affirming pelvic floor therapy is specialized physical and/or occupational therapy that addresses pelvic health while taking your gender identity, anatomy, medical history, and individual goals into consideration.
There is no single experience of being transgender, nonbinary, or gender diverse—and there is no single approach to pelvic health.
Some people experience pelvic floor symptoms related to surgery or changes in their body. Others may experience pain, urinary or bowel dysfunction, or sexual health concerns unrelated to surgery. Some simply want to better understand their pelvic floor and feel more comfortable in their body.
Who Can Benefit From Gender-Affirming Pelvic Health Care?
Gender-affirming pelvic floor therapy may benefit transgender, nonbinary, gender-diverse, and gender-questioning individuals experiencing:
- Pelvic floor dysfunction
- Pelvic, abdominal, hip, or low back pain
- Pelvic floor muscle tension or spasms
- Pain with penetration or sexual activity
- Pain with or following gender-affirming surgery
- Urinary urgency or frequency
- Urinary leakage or incontinence
- Difficulty starting or fully emptying the bladder
- Constipation or difficulty with bowel movements
- Painful bowel movements
- Postoperative scar tissue or tissue restrictions
- Core weakness or difficulty managing abdominal pressure
- Changes in pelvic function following surgery
- Difficulty returning to exercise or physical activity
- Pelvic health concerns associated with changes in hormone therapy or body composition
You do not need to have a specific diagnosis or have undergone gender-affirming surgery to benefit from pelvic floor therapy.
Gender-Affirming Pelvic Floor Therapy Before & After Surgery
Pelvic floor physical therapy can be a valuable part of preparing for and recovering from certain gender-affirming procedures.
Depending on your surgery and individual needs, therapy may address:
- Pelvic floor muscle coordination and relaxation
- Pelvic mobility
- Postoperative scar and soft-tissue mobility
- Pain and sensitivity
- Bowel and bladder function
- Breathing and pressure management
- Core and hip strength
- Functional mobility
- Return to exercise
- Sexual function and comfort
Your surgeon’s postoperative restrictions and recommendations always guide the timing and progression of therapy. We work within those guidelines to help you safely return to the activities and function that matter to you.
Pelvic Floor Therapy That Goes Beyond Kegels
Pelvic floor therapy isn’t simply about strengthening the pelvic floor.
Depending on your symptoms, your pelvic floor may need to become stronger, more coordinated, or better able to relax. We may also need to address your breathing, abdominal pressure, hips, spine, posture, mobility, or movement patterns.
Treatment may include:
- Pelvic floor muscle retraining
- Pelvic floor relaxation and down-training
- Breathing and pressure-management strategies
- External manual therapy
- Scar and soft-tissue mobilization
- Core and hip strengthening
- Mobility exercises
- Movement retraining
- Bladder and bowel education
- Personalized home exercises
- Education for returning to exercise and daily activities
Every treatment plan is individualized to your anatomy, symptoms, comfort level, and goals.
A Private & Respectful Approach to Pelvic Health
We recognize that pelvic floor therapy can feel vulnerable for anyone. For individuals who have experienced gender dysphoria, uncomfortable medical encounters, or healthcare environments that did not feel affirming, the idea of pelvic health treatment can feel particularly intimidating.
We want your experience at Magic City Physical Therapy to be different.
We listen first. We ask before touching you. We explain what we’re doing and why. And we respect your right to establish boundaries around your body and your care.
You are always an active participant in your treatment—not simply a patient being treated.
What Happens During Your First Visit?
Your first appointment begins with a conversation.
We’ll discuss your symptoms, medical history, previous surgeries or treatments, current concerns, and what you hope to accomplish through therapy.
We’ll also talk about what makes you feel comfortable during your care. If there are specific terms you prefer for your anatomy, communication preferences, or aspects of an examination that you would like to discuss beforehand, we’re happy to have that conversation.
An internal pelvic floor examination is never automatically required. If an internal assessment could provide valuable information, we’ll explain why, discuss your options, and obtain your consent before proceeding. You can decline or change your mind at any time.
You Don’t Have to Navigate Pelvic Health Alone
Finding a pelvic health provider who understands your body, respects your identity, and makes you feel comfortable shouldn’t be difficult.

Whether you’re experiencing pelvic pain, urinary or bowel concerns, sexual health symptoms, postoperative changes, or simply want a provider who understands gender-affirming pelvic health, we’re here to help.
Your identity matters. Your comfort matters. Your goals matter. Your care should reflect all three.
